Our mission is to provide specialized physical therapy evaluation and one-on-one treatment in a practical and compassionate manner.

We have elected to not be contracted with any insurance company, therefore we fall under an out of network provider designation for insurance companies. At In Control Physical Therapy, AZ LLC we provide therapy on a “fee at time of service” basis.

Prior to your first scheduled appointment, call your insurance company to completely understand your physical therapy out of network benefits. An Insurance Worksheet is available to help you ask the insurance company the right questions about your physical therapy benefits for potential reimbursement. We can provide chart notes or other documentation at your written request. Some companies may reimburse you at 80%, 60%, or 40%, and some may not reimburse you at all. We cannot make guarantees or estimates regarding what reimbursement your plan allows. Please note, we are not contracted with Medicaid and if you are medicaid eligible we are not able to provide services.

Payment is made by the patient in full at the appointment time.

As a fee for service providers, we treat patients for 1 hour for every visit and treatment is not limited to what an insurance is willing to pay for. Fee for service allows you as the patient the freedom of choice when it comes to your healthcare. You may choose to see providers outside of the network for quality care.

Treatment FAQ's

Each patient/parent provides the medical history, prior studies, and bladder or bowel symptoms. An intake questionnaire based on the symptoms is completed ahead of the initial evaluation to give the therapist information to be useful during the evaluation. The perineal region will be evaluated with the parent/patient signed consent to determine the function of the pelvic floor muscles. A plan of care is then created based on the patient clinical presentation. 

Each patient is unique and  an individualized plan of care based on the diagnosis is followed to promote healing based on the clinical findings. During their PT sessions we use a combination of tools, including exercises, therapies and modalities. Our primary goal is to create a relaxing environment where your child feels comfortable, thereby promoting cooperation in the clinic and at home to do what is necessary to improve their symptoms, a sense of healing and wellbeing.

The Initial evaluation is typically 60 – 70 minutes, and follow up sessions last 60 minutes

No. Vaginal or rectal exams are NOT done by physical therapists in the pediatric population. Visual inspection of the perineum will be conducted to observe the following:

  • skin irritation or redness from soiled underclothing
  • pelvic floor muscle contraction to see if the child understands how to use the muscles correctly
  • normal sensation and reflexes of the anus with a light touch (cotton swab used)

Surface electromyography (SEMG) biofeedback is a learning technique that utilizes specialized equipment to allow the patient to see how their pelvic floor muscles are working on a computer screen or using sound. It involves the monitoring of a life process (bio) and the return of that information to the patient and therapist in a meaningful form (feedback). The pelvic floor musculature is often difficult to feel or to know if it is contracting and if it is contracting correctly or relaxing.  

Biofeedback has been demonstrated to be very effective in the evaluation and treatment to improve the function of the muscles of the pelvic floor. These muscles play a key role in bladder and bowel control. Anyone interested in learning how to relax tense muscles, strengthen weak ones, or to control and coordinate use of muscles may benefit from biofeedback.

In the pediatric population we use external stick-on sensors (electrodes) placed near the anal opening, and abdomen. These sensors are used to monitor the muscle activity of the pelvic floor. This enables you and your therapist to see and evaluate resting muscle activity as well as evaluate the muscle contraction and ability to relax. The results of the evaluation will help the therapist design a specific treatment plan for your needs.

General FAQ's

Arizona is a “direct access” state for physical therapy services.  You do not need a referral from a physician for physical therapy services.

However, bowel, bladder, and other pelvic health problems may also occur in conjunction with other serious medical problems, and should be evaluated by your medical doctor. At In Control Physical Therapy we require a referral from your medical provider prior to your first session to ensure adequate medical screening and testing.

Our goal is to collaborate and communicate with your health care providers to ensure comprehensive treatment.

Additionally, your insurance guidelines for out of network benefits and policy may require a doctor’s prescription for you to be reimbursed.

  • Completed Patient Forms, Monica will send these and will give you the opportunity to submit them ahead of the appointment for her review
  • The patient forms take time to fill out and should be done ideally at home before you come for your first visit.
  • If you do not bring them with you, you will need to fill them out prior to seeing your therapist, however, this time will count against the 60 minute evaluation time.
  • You may bring a separate list of current medications, past surgeries, and any specific tests/ results with you if you prefer.
  • The Referral from your physician; however this will have had to be faxed directly to In Control ahead of you scheduling the appointment. if required by your insurance company and you plan to submit for reimbursement.

You should arrive 15 minutes early for your first visit. If you did not complete your paperwork you should allow an additional 15 minutes to fully complete this prior to your appointment. We suggest that you arrive a few minutes early to your follow up sessions to allow time to check in, use the restroom, and change clothing as needed prior to seeing your therapist.

Please wear or bring comfortable clothing that allows ease of movement for your first and follow up visits. We also have gowns and special shorts for use for pelvic floor therapy. Comfortable walking shoes or sneakers to allow us to better observe your mobility and walking patterns.

Patients are usually seen once a week. The number of total treatment sessions will vary according to the medical diagnosis, duration and intensity of symptoms, medical and musculoskeletal contributors, and your consistency with attendance and adherence to the home program.

At the first appointment, the therapist will review the assessment and recommend a plan of care for your consideration. The patient’s progress will be discussed throughout the course of treatment, and adjustments to the plan of care will be made based on clinical decision making. The total visit number is an estimate, it is dependent on many variables.

Our philosophy is to work with families/patients to help them succeed and meet their goals. From day one, our goal is to work with you to resolve your child’s symptoms and progress them towards improved quality of life! Ideally, patients should attend regular sessions, especially in the early part of treatment to effectively reduce symptoms and reinforce the home program. However, we understand that our patients have multiple time and financial commitments that may affect their ability to attend regular physical therapy sessions. We will do our best to balance your time in the clinic with hands-on therapy and patient education to allow you to best maintain progress between clinic visits.

Yes.  We collaborate with other physical therapists and providers who suspect pelvic floor dysfunction in their patients. We will conduct a detailed evaluation, provide a written report, and discuss follow up recommendations with the parent/patient and provider. A medical release form will need to be completed by the parent/patient.

All treatments require a parent or parent authorized caregiver to be present with the child and therapist during all evaluation and subsequent treatment sessions.
